The increasing integration of artificial intelligence systems into business workflows offers significant opportunities but also introduces new challenges, particularly concerning incident management. Recent research by ISACA, a global association of digital trust professionals, has highlighted a worrying gap: most surveyed organizations cannot explain how quickly they could stop an AI system emergency or identify the root cause of the problem.
According to the ISACA report, 59% of digital trust professionals do not understand how quickly their organization could halt an AI system during a security incident. Only a mere 21% reported being able to intervene meaningfully within half an hour. This scenario suggests a landscape where compromised AI systems could continue to operate unchecked, increasing the risk of irreversible damage to data, operations, and corporate reputation. Only 42% of respondents expressed confidence in their organization's ability to analyze and clarify serious AI incidents, a condition that can lead to operational failures and security risks. Without a clear understanding of these incidents, the likelihood of repeated errors only increases, and the inability to explain such events to regulators and leadership can result in legal penalties and significant reputational damage.
Another critical area is accountability: 20% of respondents do not know who would be responsible if an AI system caused damage, and only 38% identified the Board or an Executive as ultimately responsible. This ambiguity is particularly problematic in contexts where data sovereignty and regulatory compliance are priorities, such as in self-hosted or air-gapped deployments, where end-to-end control is a fundamental requirement. The widespread perception that AI risk is purely a technical problem, rather than an overall organizational management issue, contributes to these gaps.
Towards Integrated and Proactive AI Governance
Ali Sarrafi reiterated that the solution does not lie in slowing down AI adoption, but in fundamentally rethinking its management. AI systems, in his view, need to sit in a structured management layer that treats them as โdigital employees,โ with clear ownership, defined escalation paths, and the ability to be paused or overridden instantly when risk thresholds are crossed. This approach transforms AI agents from โmysterious botsโ into inspectable and trustworthy systems.

ISACA's research highlights a major structural issue in how AI is being deployed across different sectors, with over a third of organizations not requiring their employees to disclose where and when AI is used in work products, increasing the potential for blind spots.
Even with more stringent regulations making senior leadership more accountable, many organizations are failing to implement and use AI safely and effectively.
